Position Summary:
As a Disability Support Worker at Elevate Community Services, you will play a crucial role in supporting individuals with disabilities to lead fulfilling and meaningful lives. Your responsibilities will include providing practical assistance, emotional support, and fostering a positive and inclusive environment for our clients.

Key Responsibilities:
1. Individualized Support:

- Collaborate with clients to develop and implement individual support plans.
- Assist individuals with daily living activities, personal care, and mobility as required.
- Foster independence by promoting and teaching life skills.

2. Emotional Support:

- Build rapport and establish positive relationships with clients.
- Provide emotional support and encouragement to promote overall well-being.
- Recognize and respond to the unique needs and preferences of each individual.

3. Community Engagement:

- Facilitate community participation and inclusion for clients.
- Organize and accompany individuals to community events, social outings, and recreational activities.
- Promote social skills and encourage connections within the community.

4. Documentation and Reporting:

- Maintain accurate and detailed records of client progress, activities, and any incidents.
- Complete regular reports and communicate effectively with the multidisciplinary team.

5. Collaboration:

- Work collaboratively with other team members, including therapists, case managers, and healthcare professionals.
- Attend team meetings and contribute to the development of support strategies.

Qualifications:

- Certificate III or IV in Disability Support or a related field.
- Proven experience in disability support or a similar role.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Empathy, patience, and a genuine commitment to improving the lives of individuals with disabilities.

Pay: $41.00 - $80.00 per hour

Expected hours: 20 - 40 per week
