**Care Plan Advisor - Permanent Part Time | Immediate Start**_Mannum, SA_

Join Mid Murray Support Service (MMSS) as a **Care Plan Advisor**, supporting older Australians to live safely and independently at home.

**About the Role**:

- Permanent Part Time position with immediate start (25 Hours per week)
- Coordinate and review in-home aged care & disability services
- Liaise with clients, providers, and internal teams
- Ensure care plans align with client goals and budgets
- Advocate for clients through onboarding, assessments, and reviews

**What You’ll Need**:

- Cert III in Individual Support (Ageing & Disability) or equivalent (required)
- Aged Care or NDIS coordination experience
- First Aid + CPR, valid Driver’s Licence, own insured vehicle
- Strong admin, tech, and communication skills
