**About the Role**:
Miracle Health Services is seeking a dedicated **Care Advisor** to join our **Home Care Program** team in the **northern suburbs of Melbourne**. In this role, you will oversee a caseload of clients, advocating for their independence and enabling them to remain safely in their own homes. You will facilitate personalised discussions and create tailored care plans that align with their needs, ensuring they receive the best support through their **Home Care Package**.

**Key Responsibilities**:

- Conduct regular **home visits** to assess and support clients.
- Perform **initial assessments** for new clients, identifying their needs and making appropriate referrals or recommendations.
- Develop, implement, monitor, and review **individualised, client-focused care plans**.
- Work closely with **Care Coordinators** to organise and deliver services that align with each client’s care plan.
- Liaise with **healthcare providers, allied health professionals, social workers, and community services** to provide holistic support.
- Manage **financial aspects** of care planning, ensuring compliance with **Home Care Package funding guidelines** while maintaining quality service delivery.
- Support **case discussions, case conferences, feedback reviews, incident management, and complaint resolution**.
- Keep **accurate records** of client assessments, care plans, services provided, and any changes or incidents.
- Contribute to **teamwork and continuous improvement initiatives**.

You’ll be a great fit if you have:

- A **clinical background** as a **Social Worker, Enrolled Nurse (EN), Registered Nurse (RN), Allied Health Practitioner, or Case Manager** (highly desirable).
- Experience managing **Home Care Package clients with complex needs**.
- Strong knowledge of **safety practices** in the **Aged Care or Health Sectors**.
- Proficiency in **assessments, care planning, service implementation, evaluation, and documentation**, following **Consumer Directed Care principles**.
- Excellent **communication skills**, allowing you to liaise effectively with clients, service providers, and healthcare professionals.
- A deep understanding of **safety, equal opportunity, and legal compliance** in client care.
- A commitment to **Miracle Health Services' values of care, respect, and excellence**.

**Essential Requirements**:

- A **current Working With Children Check** (or willingness to obtain one).
- The ability to **pass a national police clearance**.
- Current **First Aid and CPR certification** (or willingness to obtain).
- Up-to-date **COVID-19 and flu vaccinations**.
- **Australian working rights**.
- A **valid Victorian driver’s license and access to a reliable, registered vehicle**.

Pay: $37.00 - $45.00 per hour

Schedule:

- 8 hour shift
- Monday to Friday
- Morning shift
- No weekends
- On call
