**Position Title: Community Care Support Worker**
**Type of Employment: Full-time**
**Award: SCHADS L3- $72,000- $77,000 per annum**

As **Community Care Support Worker, **you will work as part of a multidisciplinary care team to maximise the capacity of young people aged 15-18 years to effectively transition out of care to sustainable independent living.
We are seeking a compassionate and dedicated individual to join our Homelessness and Housing team on a** full-time** basis. This position is primarily located at our **Mansfield Park **site.
Reporting to the Community Care Team Leader, you will provide essential support and assistance to young people to develop and increase their capacity and links to the community.
Your day-to-day activities will include:
- Facilitating and contributing to the development of activities that promote effective independent living skills, such as cooking, meal planning, budgeting, shopping, general household maintenance and other daily living requirements.
- Providing direct supports to young people in a manner informed by attachment and trauma informed care.
- Working collaboratively to review and ensure each young person progresses to their identified goals.
- Providing supports and facilitating activities to support the development of parenting skills for expectant / young parents.
- Mentoring and coaching young people to assist them to achieve their goals in accordance with agreed Participation and Therapeutic Care Plans.
**About You...**
- Experience in developing case plans, case noting and report writing.
- Time management and organisation skills, able to plan, prioritise and achieve outcomes with mínimal supervision and/or direction and to successfully meet deadlines and KPIs.
- Interpersonal, conflict resolution and negotiation skills along with the initiative to identify innovative and lateral solutions.
- Strong engagement skills and ability to interact effectively and sensitively with young people from diverse cultural backgrounds.
- Excellent communication skills, both written and verbal, including the ability to write timely and accurate case notes and reports.
- Empathy and compassion, and a genuine commitment to making a positive impact in the community.
**For this role, you will need to**:
- Hold a minimum Certificate IV qualification in a relevant discipline (e.g., Youth Work, Social Work, Social Science, Counselling).
- Hold and maintain an approved First Aid and CPR Certificate.
- Be prepared to undergo a Psychological Assessment.
- Have possession of a current valid Driver’s License, willingness to drive.
- Hold and maintain Safe Environments for Children and Young People Certificate.
- Willingness to participate in an out-of-hours on-call roster is required.
