**About the Role**

Seniors Community Care is seeking an effective, innovative case manager to join our Home Care Packages (HCP) team, working to ensure that the goal of providing efficient professional service delivery of the highest standard and legislative requirements are met.

This position will be responsible for the day to day management for the provision of Case Management for Home Care Packages. To undertake the activities required to coordinate delivery of services and case management within the HCP guidelines.

**Essential Criteria**
- Tertiary qualification in Nursing: Social Work: Allied Health or other related field
- Experience within the HCP environment
- Comprehensive working knowledge and experience with HCP legislation and funding
- Current Police Record Check and Working with Children Check
- Fully Vaccinated against COVID-19 (Mandatory requirement)

Schedule:

- Monday to Friday

- Case management: 2 years (required)

Licence/Certification:

- Police Record Check (required)
- Working with Children Check (required)
- COVID-19 Vaccination (mandatory) (required)
- Victorian Driver Licence (required)
