Key Worker Targeted Care Packages

Quantum Support Services is seeking a passionate and dedicated person to join our team in assisting in transitioning eligible children and young people from residential care to more appropriate care arrangements where their care needs will be better met, and in preventing young people from entering or at risk of entering residential care., located in Morwell 3840
- About the Role:

- Contribute to the program development and ensure adherence to the relevant DFFH Registration Standards and Quantum Support Services Policies and Procedures.
- Establish and lead the care team in supporting the child or young person, including developing, implementing and reviewing care plans using the data collection template relevant to each specific program area.
- Directly engage and work with the child or young person, their family, carers and other member of the care team in a supportive, inclusive and respectful manner.
- Support the care givers in order to sustain the placement for the child or young person in their care.
- Provide support in exploring and accessing housing and community options as appropriate to the young person.
- Provide a client focused approach which takes into account the individual needs of the children and young people.
- Demonstrated understanding of Child Protection matters and legislation relevant to the Out of Home Care space.
- A demonstrated knowledge and understanding of issues surrounding the out-of home care and family services system and the available resources and services in Gippsland.
- Experience in the provision of support to children, young people and their families with multiple and complex needs.
- Valid Victorian Driver’s Licence |Working with Children Check |Police Check
- Minimum requirement of Diploma of Community Services or other appropriate tertiary qualification/s and/or experience relevant to this position (refer to Position Description).
