**About the Program**
Insync is a program for young people who are at risk of, or who are currently experiencing, homelessness in Brisbane, Redlands and Logan. Our team supports young people with crisis and transitional accommodation, while helping to increase their living skills and achieve their goals through a case planned process.
**About the Role**
We are seeking a Practitioner to join our team and provide comprehensive support to young people facing challenges such as family and domestic violence, substance use, mental health issues, and family disconnection. In this role, you will lead case management efforts alongside Youth Workers, ensuring that young people receive ongoing support even after they exit our housing services. Your work will be crucial in helping them navigate their journeys toward stability and independence.
This role is a Full-Time 14-Month Fixed-Term Contract working 38 hours per week at our Mount Gravatt East service. Contract ending December 2025 with possibility of extension pending the extension of Government funding.
Pay Rate: $49.28 per/hr - SCHADS-CAE Award Level 3
**Responsibilities**:
- Provide case management, mediation, and advocacy for young people and their families, adhering to Evidence-based practice and frameworks.
- Assist clients in setting personal goals and developing independent living skills, while coordinating access to community and government resources.
- Make referrals to specialist services and ensure integrated service delivery through effective communication and collaboration with relevant professionals.
- Contribute to the Insync Business Plan and participate in strategic reviews to address community needs and service delivery gaps.
- Maintain high standards of documentation, including reports and policies, and ensure all resources from external agencies are current and accessible.
- Participate in team meetings to share insights, support colleagues, and identify opportunities for service improvement and professional development.
**Qualifications, Credentials and Other Requirements**
- Degree in Social Work, Human Services, or a related field, or equivalent experience.
- Current National Police Check
- Current Blue Card (Working with Children).
- Valid Queensland Driver Licence and reliable vehicle.
- Willingness to work outside standard hours and provide on-call support as needed.

- Strong understanding of youth homelessness and its influencing factors.
- Experience in trauma-informed, solution-focused case management.
- Excellent relationship-building and communication skills to engage with diverse stakeholders.
- Familiarity with public and community housing systems.
- Ability to work collaboratively in a team-oriented environment.
