Care Partners Australia is a specialist provider of multicultural support services providing exceptional care and support across NSW and VIC.

We are an accredited and registered NDIS service provider offering short and medium-term accommodation and assistance for people with disabilities including adults, children, and teenagers in Sydney. We also offer a range of Specialised Disability Accommodation properties across NSW in regional and metropolitan areas.
- Provide care and support to residents in accordance with their individual needs and care plans
- Assess resident’s needs, considering their age, developmental stage, disabilities and individual circumstances
- Provide a range of support to residents including, but not limited to, information provision, emotional support and guidance for families
- Collaborate with other professionals involved in the resident’s care such as physiotherapist or occupational therapist
- Assist residents in accessing relevant services and programs such as appropriate education and training programs, or health, welfare and support services.
- Plan and facilitate recreational and therapeutic activities to promote socialisation, cognitive stimulation, and physical health
- Evaluate effectiveness of support services in meeting residents’ needs and improving their overall wellbeing, seeking feedback from residents, families, and other team members for continuous improvement
- Monitor clients physical and mental well-being within a residential facility and documenting any changes
- Participate in staff meetings, training sessions, and professional development activities to enhance skills and knowledge.

Successful applicants must have:

- Relevant qualification or work experience in lieu
- 2 years of work experience in similar role
- Ability to work well in a team
- Excellent communication skills

**Salary**: $70,000.00 - $80,000.00 per year
