**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

**About the role**
- Barwon Health in Geelong is seeking a nursing or allied health professional to join the McKellar Kids Rehab team.
- McKellar Kids Rehab is a multi-disciplinary service which incorporates the Victorian Paediatric Rehabilitation Service (VPRS) and the Young Adults Transition Service (YATS).
- The Access Navigator is an interdisciplinary role which supports young people with chronic medical conditions and/or disability to navigate the transition from paediatric to adult healthcare.
- The Access Navigator may also support the families of children and young people with complex rehabilitation needs to navigate the network of health, disability and community support systems in a key worker model.
- This role will be predominantly based at McKellar Centre with some outreach to the community and other Barwon Health sites.
- To be considered for this opportunity, you will have the following:

- A degree in Nursing or Allied health discipline
- Experience working with children and families
- Experience working with clients who have a complex or chronic health care needs
