**Purpose of Position**

The Support Worker will provide support and care to aged care consumers and participants in the home, at Chung Wah CC Hubs, in the community and as stipulated in the care plan of the service users. This may include aged care and disability support.

**Key Responsibilities**:

- The Support Worker is responsible for the delivery of programs and support that keep people safe, engaged and actively supported.
- All interaction needs to facilitate choice and control, be flexible and promote inclusion, community-based options, and fun.
- Provide care and support to consumers and participants in a professional, confidential manner, encouraging independence and active living.
- Contribute to continuous improvements, attending staff meetings, planning, and providing feedback.
- Promote safety by complying with CC WHS requirements and guidelines.
- Attend regular training sessions inhouse and online.
- Other additional duties as assigned by the Coordinator or Manager
- Maintain valid training certificates, NDIS Workers Screening Check and other certificates/ training as required to deliver safe and quality care.

**Centre-Based Duties**
- Support aged care consumers and participants at the Day Centre, duties may include personal care and supporting consumer independence.
- Assist with exercise and activity programs as part of the CC Wellness program.
- Assist the centre staff arrange day to day activities and plans at the centre.
- Provide transport service to and from Centre including outings as scheduled.

**Home Help Duties**

Adhere to the care plan and provide support and service as per the care plan.
- Report and feedback to the case worker when the care plan needs to be reviewed and updated for additional support.
- Report to the case worker changes or observations in the consumer, participant, or home environment.
- Promote and encourage independence.

**Social Support Duties - Individual or Group & Transport (when applicable)**
- Transportation to appointments, shopping, Day Centre activities, social support in the community, etc., as per assigned duties.
- Be responsible for the welfare, comfort, and safety of participants during transportation and activity sessions.

**Qualifications**:
**Essential**
- Certificate III in Individual Support is a MUST
- NDIS Worker Orientation Module: “Quality, Safety and you”
- Registration and a valid NDIS Worker Screening Check Certificate
- Bilingual and able to communicate effectively in English.
- Ability to work independently and in a team.
- Have a valid WA Driver’s License and own a vehicle.
- Relevant experience/qualifications working with CALD community groups and people with disabilities.

**Job Type**: Casual

Pay: $40.93 - $42.27 per hour

Expected hours: 2 - 38 per week

Schedule:

- Afternoon shift
- Fixed shift
- Monday to Friday
- Morning shift
- Shift work

Supplementary Pay:

- Performance bonus
