**Job Summary**:
**Key Responsibilities**:
- Develop, implement, and review personalized care plans for clients based on assessments of their needs, preferences, and goals.
- Coordinate with clients, their families, and healthcare professionals to ensure comprehensive and consistent care.
- Regularly monitor and evaluate client progress, adjusting care plans as necessary to ensure quality outcomes.
- Provide support, advice, and guidance to clients and their families on care options and available services.
- Ensure compliance with relevant policies, regulations, and standards in the provision of home care services.
- Manage client records, ensuring all documentation is up-to-date, accurate, and secure.
- Collaborate with the care team and support workers to provide seamless service and ensure continuity of care.
- Handle client inquiries, concerns, and complaints in a professional and timely manner.
- Maintain strong communication with funding bodies, government agencies, and other external stakeholders.
**Qualifications and Skills**:
- Bachelor's degree in social work, nursing, healthcare management, or a related field.
- Minimum 2 years of experience in case management, preferably in the home care, aged care, or disability care sector.
- Strong understanding of home care services, funding models (such as NDIS or Home Care Packages), and relevant regulations.
- Excellent interpersonal and communication skills, with the ability to build relationships with clients, families, and care teams.
- Strong problem-solving skills and the ability to work independently while managing multiple cases.
- High level of organization and attention to detail.
- Proficiency in case management software and Microsoft Office Suite.
- Current driver’s license and willingness to travel for client visits.
**Desirable**:
- Experience with managing self-managed and plan-managed clients.
- Knowledge of local healthcare resources and support services.
