About the role

ABOUT THE ROLE

Therapeutic workers across The Glen Group supervise clients in line with routine, provide individual case management & support work where necessary. The Glen is a challenging, but supportive environment that puts clients needs first.

This is a full-time role that requires some flexibility i.e. evening, sleepover and weekend work.

CASE MANAGEMENT RESPONSIBILITIES
- Plan, organise and schedule case management sessions with clients.
- Understand and implement early exit planning
- Undertake holistic and culturally sensitive assessment of client needs (in line with ADARRN Model of Care). This includes working collaboratively with the client to develop interventions to motivate clients toward change
- Undertake routine client outcomes measures (COMS) and progress monitoring with client
- Complete case management plans using client management system
- Assist with all court proceedings (Children’s, Family, Criminal, etc) and prepare progress reports, when required
- Provide aftercare support and following up with clients after they’ve completed the program
- Provide support and advocacy on behalf of the client
- Ensure the case management team are referring clients to the most appropriate services or clinicians (such as grief counsellors, psychologists, financial counsellors, gambling counsellors etc.)

CLIENT RESPONSIBILITIES:

- Facilitate and assist in the delivery of reflective, education and support groups
- Crisis management: Work collaboratively with management team in times of crisis, including communicating to staff, clients and board.
- Support the clinical team to make informed client decisions, based on facts.
- Provide training & mentoring to the clinical team regularly (with COO & Clinical Lead)
- Help drive case management projects such as the employability project.

SUPERVISORY RESPONSIBILITIES:

- Plan, organise and deliver activities (according to the daily routine) and group therapy sessions for clients.
- Ensure clients are complying with house rules at all times.
- Assist clients exiting the program under a disciplinary discharge

ADMINISTRATION RESPONSIBILITIES
- Maintain client case notes, records, and reports as required including appropriate and timely clinical documentation, recording of treatment results, care plans, outcomes, and recommendations in the client file on the client management system.
- Ensure case notes based on observable behaviour and facts
- Actively contribute to staff meetings, team development activities, performance initiatives and service development activities.
