About You
The purpose of this position is to deliver a high-quality support service to young people who are transitioning from Care. The role will focus on supporting young people while they are preparing to leave care. The role will also focus on building relationships with external service providers and informal community supports that support young people in the community, this may include faith, arts, or cultural groups that help young people maintain positive cultural and social connections. The position is also required to work closely with internal colleagues such as Out of Home Care (OOHC) caseworkers and community engagement staff.

Key Responsibilities
- To support with MCFP Aftercare model development which will offer young people a continuum of care from family preservation to independence at 21 years of age.
- Produce work that is consistent with SSI legal and contractual responsibilities, expectations and values. This includes establishing effective professional relationships with internal and external partners to the benefit of young people preparing to leave care.
- Monitoring the ongoing outcomes for children and young people who have left SSI’s care and reporting on progress and outcomes of young people to leadership staff as required.
- Maintaining contact with young people and their supports in exploring and identifying the underlying issues that have given rise to the need for support and work with young people to help overcome barriers to achieving their goals for independence and long term emotional, financial and social wellbeing.
- Work closely with young people, carers and OOHC caseworkers to develop and implement leaving care plans and participate in discussions to identify potential skills needed for young people to live successfully independently. Provide advice to caseworkers when developing leaving care plans.
- Ensuring program reports and data to internal and external stakeholders are completed as required and as requested.
- Develop and deliver After Care topics to empower the young people and prepare them to leave care and undertake consultations with young people on an annual basis (minimum).
- Attending individual supervision and team meetings with relevant programs and participate in individual performance and development plan and to support, buddy and guide caseworkers during the leaving care process.
- Develop and negotiate partnerships/strong relationships with DCJ, Education, Juvenile Justice, Police, Housing and Employment for better outcomes and participate in sector forums on independent living

Essential Criteria
- Tertiary qualifications (Bachelor Degree) in Social Work, Psychology or other relevant Human Services area (e.g. Teaching, Youth Studies
- Commitment to, and understanding of, culturally competent practice and service delivery.

- Ability to manage crisis related behaviour with carers and knowledge of services and support.
- Demonstrated understanding and capacity to high level case management, rapid rapport and engagement strategies and strong understanding of Child Protection and permanency care systems
- Well-developed skills, including report writing, case notes and the use of a range of electronic data systems and time management skills.
- Demonstrated skills in working with young people who have multiple and complex needs within a range of settings including Homelessness, relationship issues, Alcohol or other drug misuse, disability and behavioural challenges
