**Position Overview**: We are seeking experienced and dedicated Support Workers to provide care for a participant in Glebe on Mondays, Tuesdays and Wednesdays between 6:30 a.m to 8:30 a.m. The role involves working with our client in their home located in Glebe. This role is under contract, and the worker must have an Australian Business Number (ABN).

**Key Responsibilities**:

- **Client Care**: Provide high-quality support to clients with complex needs, ensuring their physical, emotional, and social well-being.
- **Daily Living Assistance**: Assist clients with daily living activities such as personal care, meal preparation, medication administration, and mobility support.
- **Individualized Support Plans**: Implement and adhere to individualized support plans, ensuring the delivery of tailored care that meets the specific needs of each client.
- **Community Engagement**: Encourage and support clients to participate in community activities and programs, promoting social inclusion and engagement.
- **Documentation**: Maintain accurate and up-to-date records of client care, including progress notes, incident reports, and care plans.

**Qualifications**:

- **Experience**: Previous experience working with complex client cohorts, particularly in a clinical or disability services setting, is essential.
- **Education**: A certificate or diploma in Disability Services, Community Services, Nursing, or a related field is preferred.
- **Skills**: Strong understanding of the needs and challenges faced by individuals with complex disabilities.
- Excellent communication and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
- High level of patience, empathy, and a genuine desire to improve the lives of individuals with disabilities.
- **Requirements**:

- Current First Aid and CPR certification.
- Valid Working with Children Check and National Police Check.
- Reliable transportation and willingness to travel to various client locations.
- Must have an Australian Business Number (ABN).
