Make a difference supporting people experiencing or at risk of homelessness.
Join **Communify’s HART 4000** team and help individuals find stability and sustainable housing through a **Housing First** and **trauma-informed** approach.

**About the Role**

As part of our **Housing First** and **trauma-informed** service, you’ll:

- Conduct intake, assessment and case management with individuals and families
- Manage a caseload of around 10-12 participants, plus drop-in crisis assessments on rotation
- Develop and deliver client-focused case plans to achieve sustainable housing outcomes
- Provide referrals and broker supports to meet immediate and long-term needs
- Build partnerships with housing providers, government agencies and community services

**About You**

You’re passionate about social justice and skilled in supporting people facing housing instability. You’ll bring:

- Relevant tertiary qualifications (e.g. Social Work, Community Services or related field)
- Demonstrated case management experience using a strengths-based approach
- Excellent communication, time management and problem-solving skills
- Ability to respond effectively in crisis situations
- Strong teamwork and collaboration skills
