Your part in our UnitingCare team
We are currently seeking to appoint a Case Worker to work as a part of The Gardens Supported Accommodation Program. Our Case Workers support families experiencing or at risk of homelessness with housing case management whilst they are in short-term accommodation.
As part of this role, you will:
- Deliver rapid and coordinated assistance to people in critical need of housing and/or support through triage, assessment, case planning and crisis intervention.
- Respond and action referrals identified during case management with families.
- Adhere to program guidelines regarding the appropriate use of allocated funds.
- Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate records within the appropriate timeframes.
- Identify and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff.
- Positively represent the organisation when engaging with all stakeholders.
- Undertake other duties as and when required within the scope and capabilities of the role.
What makes a difference for us?
- You must have excellent verbal communication skills that enable you to assess needs accurately and extensively as well as transferable written communication skills to record assessments on the database.
- Ability to actively engage with people who have a range of needs and to respect the cultures of all people you support and work alongside.
- You must have a tertiary qualification (Diploma minimum) in human services, community services, youth services or related field.
- Ability to work collaboratively as part of a team as well as responsibly within an autonomous setting.
- Demonstrated competence with technology, particularly Microsoft Office and tools for data entry.
In addition to the above you will also be required to have:
- Working with Children Check (Blue Card)
- Current Open Drivers Licence.
