Penrith, Australia Bloom Disability Network Pty Ltd. Full time

Bloom Disability Network is seeking two enthusiastic, knowledgeable, skilled and RELIABLE Disability Support Worker to support a middle aged patient with mid-late stage early onset Dementia for an immediate start.

The care provided will be in-home support, transport to and from medical appointments, and community access activities.

The successful applicant will need to be **extremely **reliable, punctual and professional, and have extensive experience and a Cert III - Cert IV in Disability Support.

**MUST BE AVAILABLE**:
**Tuesday 2.30pm - 8.30pm**
**Thursday 2.30pm - 5.45pm**
**Friday 6.30am - 2.30pm**
**Friday 2.30pm - 830pm**
**Friday - 8.30pm - 6.30am (passive overnight)**
**Sunday 7am - 5pm**

**WILLING TO BE ON CALL.**

**Duties will include**:
Assistance with Personal Care: eg needing assistance with getting into the shower, helping them to dress, helping them to brush teeth, helping them to brush hair

Home Care: Helping them to navigate the home eg. finding bedroom, finding bathroom

Supervising: Patient cannot be left alone or else she can compromise her safety

Meal Preparation: Helping with cooking

Community Access: go to the movies, go to the library etc.
PAY RATE:
Weekdays: $35.50 per hour PLUS super
Sunday: $55 per hour PLUS super

**What we need from you**:
Cert III - Cert IV in Disability Support
Experience working with those with disabilities
Current First Aid
Drivers License
Working with Children Check
National Police Clearance
Two professional references

- 8 hour shift
- Night shift
