**About the Role**:

- **Full Time | Monday - Friday | 9:00am to 5.00pm**

**Care Coordinator & Administrator Position**
- Communicating between staff, patients and their families
- Developing patient goals and monitoring progress
- Maintaining patient confidentiality and deliver quality care
- Nursing duties that include clinical procedures e.g. dressing, vital signs, medication management
- Provide high quality care in line with clinical aged care standards.
- Provide detail care plans that meet individual client needs
- Manage and supervise team of care workers

**Administration Duties**:

- Maintain accurate documentation and support practice compliance
- Assist team with rostering
- Answering phone calls
- Assist with stock management and general office task

**About You**:

- Enrolled Nurse
- Current APHRA Registration
- Must have a car with current Australian driver's license
- Excellent customer service skills
- Ability to multitask in a busy environment
- Great Interpersonal and communication skills - written and verbal
- Experience working in Home Care and NDIS support.
- Experience with the Microsoft office 365 including Word, Excel, SharePoint and Outlook, with an ability to learn Right at Home programs.
- Adherence to safety and workplace policy and procedures
- Previous experience with Visual Care - an advantage
