Empower our customers to live their best life by becoming an **RN / Clinical Care Facilitator** in the **Southern Highlands **region.

**About the role**

As a Registered Nurse/Clinical Care Facilitator with BaptistCare at home you will be responsible for coordinating the delivery of high-quality care and services that support our customers to remain living at home. You’ll develop genuine connections with our customers and their loved ones and assist them to implement innovative strategies that will optimise their health and well-being and support their independence. You will manage a caseload of clients and be responsible for the clinical care and governance in our **Southern Highlands region.**

A typical day can include:

- Providing guidance and clinical support to Care Facilitators caring for HCP clients
- Working alongside our customers to set individual goals that are realistic and achievable
- Implementation of the latest best practice and evidence-based approaches
- The position is a home office based with support from local site office

To be shortlisted, you will need:

- Registered Nursing qualifications with current AHPRA registration
- Current clinical skills with experience in acute or aged care sector
- Current NSW/ACT Drivers Licence
- Demonstrated ability to work as part of a team
- Sound written & verbal communication, and computer skills.
