The Bolton Clarke Homeless Persons Program actively seeks to engage and offer a holistic primary health care response to individuals and families who are at risk of or who are experiencing homelessness. The program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services.

And we’re hiring We’re looking for a Clinical Team Manager to join our hard-working team based out of our Port Melbourne office. In this role you’ll provide leadership, role modelling and support to our HPP Community Health Nurses and will assist them to deliver direct and comprehensive nursing care to homeless and at risk people.

**‘A week in the life’ -**
- Providing leadership and vision to your team as well as strengthening key stakeholder partnerships and relationships
- Translating organisational goals into program area goals and promoting an integrated and client centered approach to care
- Ensuring all performance measures and KPIs are achieved
- Business planning, forecasting staffing and working within allocated budgets
- Building and maintaining professional networks and working relationships with stakeholders as required in order to establish, plan and promote optimal service delivery
- Assisting our Community Health Nurses to develop advocacy strategies, liaison and support networks with people experiencing homelessness, their communities, service providers and government departments to ensure the provision of quality nursing care

**What do we need from you?**
- A minimum of at least 5 years post graduate RN experience as well as management experience
- In depth knowledge and experience of homelessness as well as experience dealing with drugs/alcohol and mental health
- Highly developed clinical skills along with excellent communication skills
- Current nursing registration with AHPRA
- Australian Working Rights/Current Australian drivers licence, own vehicle required
- If appointed, a satisfactory Police Check will be required along with a working with children check
