**About the Opportunity**

We currently have an exciting opportunity to join the **Metropolitan Youth Health (MYH)** team as a** Youth Health Worker,**where you will be responsible for:

- Engagement of young people who are disadvantaged and/or vulnerable to access and navigate through specialised primary health care services
- Development and delivery of quality outcomes for clients through the provision of youth health care services within a case management framework, aligned to the Metropolitan Youth Health Model of Care

**About You**

To be successful in this role, you will have the ability to:
- critically analyse and respond to the changing needs of the clients and the organisation.- lead collaborative work activities, prioritise workloads and meet set timelines, whilst working under mínimal supervision.
- develop and deliver training and development programs
- provision of a high standard of assessment, case planning and health care services within a case management approach to young people with complex needs
