**Case Manager MSTS (Mobile Support and Treatment Service)**

**About the Mental Health Division**
The Mental Health Division provides mental health assessment and interventions for people experiencing mental illness, and their family and carers. Working from a recovery-orientated model, the Mental Health Division provides an array of hospital-based, community and specialist services for infants, children, young people, adults and aged people throughout Victoria. All mental health services work within a clinical framework that promotes recovery-orientated practice and supported decision-making.

**About the role**
As a Case Manager, you will work collaboratively with the MSTS Consultant Psychiatrist and the team to provide the highest level of clinical care to the consumers
A regular day will see you:
- ** Undertaking assessment and case management of NEMSTS consumers and contribute to the implementation, monitoring and evaluation of treatment objectives**:
- ** Deliver high standard quality care for a clinical caseload, as well as consultation and liaison with family and organisations**:
- ** Providing assertive mobile outreach for consumers in their local community**:
- ** Medication supervision to the consumers in the community**:
- ** Together with the Consultant Psychiatrist and Registrar and the consumers planning clinical interventions that are appropriate for consumer and making sure implementation is done to the high standards**:
- **Providing knowledge and skills, based on professional background, as part of a multidisciplinary team, consult with other mental health staff on specialist and community psychiatry matters**

You will be someone who has excellent written and verbal communication skills, and the ability to work both autonomously and as a part of the team.
