We are looking for a Home Care Coordinator who is committed to providing exceptional customer-focused care, with excellent communication and organisational skills.

**Position Responsibilities**:
As a Home Care Coordinator, you will be responsible for supporting clients to develop and review their Client Goal/Care Plan, coordinating and monitoring service delivery, and ensuring a high degree of client satisfaction. You will also maintain communication with caregivers, family members, and advocates, support and mentor home care workers, and provide regular case management and training to our Personal Care Workers.
