**About the role - 68.4 Hours Per Fortnight until July 2025 (Southern Adelaide - Christie's Beach)**

The Community Connections Program is funded by the Department of Human Services (DHS). The program aims to strengthen social inclusion of individuals within their communities in Eastern, Northern, and Southern Adelaide.

The Case Manager will provide case management, referral services and related support services to support vulnerable people aged 18-64 (or 49 years for Aboriginal people) living in social isolation to engage with, and participate in, community activities, groups, and volunteering opportunities.
- Key responsibilities will include, but will not be limited to:_
- Complete professional intake and assessment of clients
- Create and manage actionable care plans with clients, integrating information provided at referral and gathered through client assessment
- Connect clients with purposeful partners

**About you**:
You’re passionate about supporting vulnerable people and making a tangible impact in the community. With your experience in case management or a related field, you thrive in helping individuals break through social isolation and reconnect with community life.

If you’re ready to make a difference in the lives of those experiencing social isolation and have the skills to connect people to opportunities that enhance their quality of life, this role is for you
- You will also have the following skills and experience:_
- Tertiary qualifications in Human Services (e.g. Social Work, Youth Work, Psychology, Community Development) or Diploma in a related field of study.
- Extensive relevant experience may be considered in lieu of qualifications.
Experience in case management focused service delivery
- Experience in the provision of individualised services to support people who are disadvantaged or at risk.

**Specific Requirements**:

- You will be required to have, or be willing to obtain:_
- Full unrestricted South Australia Driver’s Licence
- DHS Child-Related Employment Screening or Working With Children check
- National Police Clearance
- Training: Provide First Aid (HLTAID0011)
- Training: Safe Environments for Children and Young People (Through Their Eyes)
- Ability to work outside of normal office hours may be required
