About the role
This full time role involves processing referrals, assessing client needs, and coordinating services to ensure a smooth and appropriate intake process. The Intake Officer acts as a key point of contact for clients and plays a crucial part in connecting individuals with the necessary in home support services.
**Your main duties will include**:
Processing and managing referrals through My Aged Care
Evaluating client needs through phone screening and assessments
Providing information about Commonwealth Home Support Programme (CHSP)
About you
Ability to take initiative, work independently and as part of a team
High level of attention to detail and strong organisation and communication skills
Efficient, confident and hardworking
Experience within the aged care sector
Understanding of CHSP and other relevant programs
