**About the Opportunity**

At Uniting, our Case Managers are called Community Support Partners. This role is responsible for the case management of clients who have complex care needs, making a difference in the delivery of quality and collarborative Home and Community Care services. The Community Support Partner will utilise specialist strategies that are person-centred, to produce outcomes that enable clients to effectively participate in decision making related to their care, access services, and to keep them engaged in their care journey.

This is a Part-Time, Fixed Term role, based at either our **Moruya or Narooma** office.

**_Aboriginal and Torres Strait Islander clients are over represented in this program. Appropriate cultural knowledge and understanding is important to deliver effectively in this role. Aboriginal and Torres Strait Islander people are encouraged to apply._**

**About you**
- Tertiary or Diploma qualification in a relevant field, such as Aged Care, Health, Business and/or working towards or equivalent years of experience
- Current Australian Health Professional Registration Authority (AHPRA) and/or Case Management Society of Australia and NZ certificate, in line with qualification/s
- Experience in the Aged Care and/or the Health environment
- Significant understanding for client needs, to convert these into positive service delivery outcomes
- Ability to effectively plan and manage budgets
- Confident in building and maintaining strong working relationships with internal and external stakeholders
- Passion for social change and contributing to an organisation of influence for vulnerable, at risk and disadvantaged members of the community
- First Aid Certificate
- Current driver's licence and access to a comprehensively insured vehicle
