**About the role**

As a Dementia Advisor with BaptistCare at home, you will be responsible for the provision of information and support to clients living with dementia and their families/carers. You will also advice to BaptistCare at home employees and other service providers to support them to deliver person-centred, quality care to clients living with dementia.

**Key Duties & Responsibilities**:

- Providing information about dementia, available services and resources to people living with dementia, their families and carers
- Using the Well-Living approach to develop individualised care plans that reflect the client’s needs, goals, preferences, and promote independence
- Equipping clients, families and carers with the information they require to exercise autonomy and make informed decisions about their care and services

**Qualifications, Skills & Competencies Required**:

- Relevant tertiary qualifications in Aged Care/Dementia Practice or equivalent qualification, or equivalent knowledge and experience
- Experience providing support to, and advice about dementia to people living with dementia, their family and carers
- Experience in the provision of counselling and other support activities
- Experience delivering client and staff education
- Comprehensive knowledge about dementia, including key features and potential implications trajectory
- Innovative service provision and ability to facilitate and drive change
- Demonstrated ability to network and consult with a diverse range of stakeholders
- Excellent written and verbal communications skills
- Effective time management and organisational skills
- Demonstrated computer literacy and willingness to utilise required software
