**About Aged Persons Mental Health Teams**

The community teams have six sub-teams with in two larger teams which are based in Kingston (known as Middle South APMHT) and the other is based in Endeavour Hills (known as Dandenong APMHT). The service provides assessment and treatment for people aged 65 years or over, who have a psychiatric disability and/or dementia with associated behaviours of concern requiring specialist treatment.

**About the Role**

As a member of a multidisciplinary team, our mental health clinicians are responsible for a range of clinical mental health services including:

- triage
- intake
- comprehensive mental health assessments including mental state examinations and risk management
- care planning and treatment
- clinical case management
- education
- discharge planning

The position also provides discipline specific services to clients as required.

Additionally you will possess:

- AHPRA registered Occupational Therapists or
- Qualified Social Workers (preferably accredited with AASW) or
- AHPRA registered Nurses (with post graduate qualifications in Mental Health)
