Care Manager

**Your Role**

You'll be responsible for planning, managing and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.
- As a Care Manager you will: _
- Manage and coordinate care services to ensure the highest standards of care.
- Develop and implement care plans tailored to individual client needs.
- Liaise with clients, families, and healthcare professionals to ensure comprehensive care.
- Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
- Ensure compliance with all relevant legislation and organizational policies.
- Monitor and evaluate care services to continuously improve quality and efficiency.
- Monitor the budget of Home Care Packages and ensure clients understand their funding and fees.

**For Success you need to**:

- Demonstrate experience in case management and client care coordination in a Community Aged Care setting
- Demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
- Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
- Current Police Screening Certificate
- Current First Aid and CPR Certificate
- Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
- Current Queensland driver's licence
- Tertiary Qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
