**Clinical Case Manager - NDIS**
- Adelaide-based | Flexible Working Arrangements_
We are seeking a dedicated Clinical Case Manager to join our team in Adelaide. This role is ideal for a Registered Nurse with experience in care coordination and a passion for supporting participants who receive funded supports through the NDIS.
With a strong participant-centered approach, you will work closely with individuals to develop and implement effective care/support plans, while also overseeing staff assessment and training to ensure compliance with industry standards.
While flexible working arrangements are available, some onsite attendance at our center is required for compliance-related tasks.
**Key Responsibilities**:
- Manage participants care/support plans while delivering a high-quality customer experience.
- Develop and implement care/support plans aligned with participants' individual needs.
- Oversee the successful delivery of participant outcomes, including conducting reviews as required.
- Ensure compliance standards are met in line with company policies and procedures.
- Maintain accurate and up-to-date documentation, including case notes and compliance records.
- Assess and train staff to meet compliance standards.
**About You**:
To be successful in this role, you will demonstrate:
**Qualifications**:
- Relevant tertiary qualifications in Nursing or a related field.
- Current AHPRA Nursing Registration.
**Skills & Experience**:
- Strong customer focus with excellent interpersonal and communication skills.
- Self-motivated with the ability to prioritize tasks and meet deadlines.
- A proactive approach to learning, adapting, and driving change.
- Ability to work effectively as part of a collaborative team.
- Minimum 12 months of experience in a similar role.
- Knowledge of the NDIS framework.
**Mandatory Requirements**:
- NDIS Worker Screening Check.
- Working with Children Check.
