At Aveo, we are seeking a
**Case Manager **to join our team at
**Aveo Albany Creek **on a permanent, full-time basis. In this role you will support residents of Aveo retirement communities to live well by working in partnership to identify care and support services required to promote enablement, dignity, independence and choice. The Case Manager will be responsible for providing information and advice about care and support services, facilitating people to access care and support services and ultimately case management of government funded and privately funded home care packages.
**About The Role**:
- Deliver excellent customer service to residents, families, staff, management, visitors and external health professionals;
- Profiling and Referral for Assessment
- Home Care Package Management
- Establish Home Care Agreements and ensure all compliance and care governance documentation is completed accurately and timely;
- Ensure all practices meet Aged Care Quality and Safety Commission Standards with the provision of safe and quality care and support services.
- Monitor the effectiveness of service provision by ensuring consumers receive the care and support services they require to meet identified needs, goals and preference;
- Ensure compliance with the scheduled Review process and conduct in partnership with consumer and/or representatives;
- Monitor consumer satisfaction with all care, support and service provision;
- Liaise with consumers general practitioner and other health professionals to promote positive outcomes;
- Complete proactive referrals to health professionals as per assessed needs;
- Liaise with brokered providers about service quality;
- Maintain strong relationships with internal and external stakeholders
- Act in accordance with relevant external legislation and internal policies that relate to this position and the organisation;
- Participate in and contribute to quality improvement processes. Provide feedback and improvement suggestions to the Group Manager and Regional Home Care Manager where identified.
**To be successful in this role you will have**:
- Minimum 3 years' experience in a case management role
- Cert IV or higher in community care related or case management qualifications; or Allied Health Professional (currently registered with the Australian Health Practitioner Regulation Authority);
- Current valid Australian drivers' licence
- Current COVID-19 Vaccinations
- Strong understanding of home care industry and Aged Care Quality and Safety Standards;
- Well-developed interpersonal and communication skills
- Prior experience in community management or business management
- Proficient in the use of technology
