**Role Description**

**Transition Care Program (TCP) **provides short-term care to optimise the functioning and independence of older people after a hospital stay. The care is goal orientated, time-limited and therapy focused. The team achieve their goals through case management, targeted therapy and implementation of services based on individually assessed needs to maximise function and independence. Exercise Physiologists play an important role as physical activity and exercise specialists as an integral treatment for primary and secondary prevention and the treatment of chronic disease. As a member of the TCP team, the EP will conduct high quality functional assessments, negotiate care plans, and review progress for clients.

**The Team**
- The Clinical Operations Division encompasses acute, allied health, cancer and mental health services. We provide a wide range of general medical, surgical and specialty services including but not limited to; Oncology, Cardiology, Renal, Emergency, Women’s and Children’s, Critical Care, Specialist Clinics, Cancer Clinics and Mental Health Services. Our Allied Health teams provide a diverse range of programs and person centred care in inpatient, outpatient, community, home and residential care settings.

**Selection Criteria**
- Essential : Degree in Human Movement or Exercise Science with post graduate Exercise Rehabilitation qualifications or equivalent 4-year exercise physiology degree. ESSA accreditation as an Exercise Physiologist.
- Essential : Commitment to ongoing professional development, and ability to meet the requirements of the Bendigo Health Allied Health Credentialing and Scope of Practice document
- Desirable : Demonstrated understanding of the impact of hospitalisation on older people, with the ability to recognise and respond to changes in the client's ability to maintain functional independence
- Desirable : Demonstrated capacity to function independently and also within an interdisciplinary team
