**Introduction**:
Exciting Permanent/Part-time (0.8 FTE) role working Monday to Thursday 9:00am to 5:00pm.
Outreach role working within the Frankston / Mornington Peninsula.
Windana. Providing connected services to more people in need.
We provide services across Victoria for adults and young people experiencing alcohol and other drug harms, family violence, mental health challenges and social disadvantage. Our Model of Care puts clients at the centre of what we do to create positive change in people's lives. We work with families, communities and other organisations to deliver trauma informed, equitable, culturally safe and integrated services based on evidence and practice wisdom.
About the program
The Aboriginal Wellbeing Program is funded by the Southeast Melbourne Primary Health Network in direct response to alcohol and other drug (AOD) harms in Frankston Mornington Peninsula’s Aboriginal and Torres Strait Island community.
The Aboriginal Wellbeing Worker works collaboratively with the community, key leaders, consumers and carers to increase awareness of the impact of AOD misuse, enhance awareness of referral pathways for support and create capacity individually, and as a collective, to improve overall wellbeing. The aim of the program is to consider the safety and wellbeing of individuals, families and communities engaging in AOD treatment.
About the role

Outreach to community utilising existing resources and identified connection points.
Develop local pathways of support across sectors relating to identified health and wellbeing needs of Aboriginal and Torres Strait Island community members who are impacted by alcohol and other drugs.
Build an understanding of recovery and harm minimisation and available supports through psychoeducation (individual and group) and community engagement activities.
Provide brief assessment, support, and referral on an outreach basis to consumers and their family members in their own environment.
Undertake supported referrals to relevant specialist services and provide support and advocacy to overcome barriers to access.
Actively participate in developing and implementing innovative models of service delivery in response to community needs.
Provide secondary consultation and training and education/ information sessions in relation to alcohol and other dug issues and broader wellbeing needs, engaging specialists to support this where required.
