**About The opportunity**:
Cosy home care has a tremendous opportunity for mental health support works in Melbourne to join our vibrant team and provide holistic care and support to 10 NDIS participants within a purposeful residential aged facility.

The uniquely designed purpose-built facility is located, and easily accessible. Our facility supports residents living with a mental health diagnosis and or an acquired brain injury to live life to their fullest potential. Our clients have a history of homelessness or have been at high risk of becoming homeless. Our dedicated team strives to provide quality individualised care to a diverse group, we are opting to recruit staff in which are able to carry out our vision and mission to achieve the overall purpose.

Mental health support workers are a vital part of providing vulnerable people with care and are able to radically improve their quality of life, providing support despite the complexities involved. The primary responsibility is to assist individuals who are unable to live independently by offering them the practical help and emotional support needed on a daily basis. Adopting a person-centered approach as such will enable clients to reach their full potential and live a fulfilling life, all whilst residing in a shared accommodation facility.

We are offering a set roster. As a successful applicant, you will be assigned to a specific client and provided with fixed number of shifts per week. The rosters will contain a combination of weekday and weekend shifts as well as flexible hours. This position is part-time and permeant with extensive benefits involved.

**Duties & Responsibilities**:
This role will involve:

- Working with NDIS participants with mental health and psychosocial disabilities, clients have vast complexities and require support with everyday activities
- Assisting individuals living with mental health illness, participate and engage in social and community activities.
- Delivering quality and client-focused supports in a highly responsive manner
- Assist participants with maintaining accommodation and being in a shared living space
- Supporting clients enhance independent living skills
- Providing psychosocial support and companionship
- Working effectively from an NDIS plan, to achieve identified goals and to facilitate client’s needs efficiently
- Adhering to policy and procedures of our organisation

To be eligible for this position, you **must**:

- have relevant qualifications in Mental Health or Allied Health Fields (diploma level or above)
- Minimum 2-3 years of experience in mental health and psychosocial disability
- Experience in a similar role with housing experience (highly desirable)
- be able to drive and own a functional working vehicle that you are happy to transport our clients in.
- have full comprehensive roadworthy vehicle insurance
- be able to support clients to meet their individual goals.
- be able to develop skills and encourage independence & healthy relationships.
- Good understanding of how to provide supports in a recovery oriented, strengths focussed practice model.
- An understanding of the National Disability Insurance Scheme (NDIS) and the nature of psychosocial distress.
- hold current Working With Children's Check
- hold current valid First Aid Certificate
- Own your own car and have full comprehensive roadworthy vehicle insurance.
