We are looking for a people-focused Clinical Care Partner with a passion for working in the community to join our HomeServe team delivering homecare packages across our residential living facilities. With locations in Tasmania, Victoria, New South Wales and Queensland, we are looking for our new team member in the Brisbane area.

**About the role**

In this dual role, you will be responsible for the home care coordination of approximately 40 clients, promoting healthy ageing and increasing their ability to stay in their home, as well as the provision of high-quality clinical assessment, advice and support around client care and compliance with relevant legislation, guidelines, practices, policies, and procedures.

**Key responsibilities include**
- Alongside the HomeServe Manager, lead the Care Partner team while providing clinical supervision and support
- Ensure suitable support services are provided that will improve quality of life and facilitate community re-engagement
- Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
- Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options

**About you**

To be successful in this Care Partner (Clinical) role you will be a Registered Nurse with a passion for working in the community. You will bring excellent communication, assessment, care planning, monitoring and customer service skills.
- Bachelor of Nursing, and hold and maintain current NMBA and AHPRA registration
- Have experience as a Case Manager or Service Coordinator of home care clients
- Strong understanding of Home Care Packages including the funding elements and Aged Care Quality Standards
- Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
- Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
- An energetic and enthusiastic clinical leader who inspires and supports their team
