As a Social Worker at Phoenix 360 Degree, you will provide professional support to individuals, families, and communities experiencing complex challenges. You will assess client needs, develop care plans, advocate for access to essential services, and work collaboratively with multidisciplinary teams to promote independence, dignity, and overall wellbeing. This role requires strong interpersonal skills, cultural sensitivity, and a trauma-informed approach to care.
Key Responsibilities
Conduct psychosocial assessments and develop individualised support and care plans in collaboration with clients.
Provide counselling, case management, and advocacy to individuals facing mental health issues, disability, housing instability, family breakdown, or trauma.
Connect clients with appropriate community resources, support services, and government programs (e.g., NDIS, My Aged Care).
Work collaboratively with families, carers, healthcare providers, and allied professionals to ensure integrated and client-focused care.
Maintain accurate and timely case notes, records, and reports in line with organisational policies and privacy regulations.
Participate in team meetings, supervision, and ongoing professional development.
Contribute to program development, service evaluations, and continuous improvement initiatives.
Promote cultural competence, inclusivity, and safety, especially when working with Aboriginal and Torres
Strait Islander communities and people from diverse backgrounds.
Key Selection Criteria
Bachelor’s or Master’s degree in Social Work
Demonstrated experience supporting clients with complex needs, including mental health and/or disability.
Understanding of trauma-informed care, person-centred practice, and the NDIS framework.
Excellent communication, negotiation, and advocacy skills.
Strong organisational skills and the ability to manage a caseload independently.
Proficiency in using case management systems and Microsoft Office Suite.
Current Working with Children Check, National Police Check, and valid driver’s licence.
Pay: $76,515.00 - $85,000.00 per year
