**The Role**:
We are seeking contractor Dietitians to join us in the **Southwest**& **Western Sydney**regions, including **Wentworthville, Canley Heights, Bankstown**or **Fairfield**areas. Responsibilities include:

- Greeting of patients and responding to their enquiries.
- To conduct individualised nutritional assessments and provide professional advice based on assessment findings.
- Individually tailored plans with evidence-based intervention within your scope of services.
- Recommendation of patients to other multi-disciplinary service providers such as Doctors, Specialists, Rehabilitation Providers, Physiotherapists, Psychologists, Podiatrists, Massage Therapists or Legal advice.
- Provide group education to raise awareness of healthy eating to the public.
- Completion of clinical notes at the end of each work day, and to provide prompt written reports/progress letters/treatment requests to the relevant parties involved.
- Provide face to face and occasional tele-support to clients in community settings. Specifically in the home or in the community, group homes, day programs.
- Recommendation of patients to other multi-disciplinary service providers such as Physiotherapists, Exercise Physiologists, Occupational Therapists, Psychologists, Speech Pathologists, Dietitians or Podiatrists.

**About You**:

- Excellent communications skills and the ability to build rapport with clients
- Empathy and patience for your clients and a passion for supporting people with disability
- A problem solver with a can-do attitude
- Proven experience, or passion in delivering individually tailored nutrition programs to a diverse member population (including Medicare, NDIS, Aged Care)
- APD accreditation
- Eligibility for registration with Medicare
- Professional indemnity insurance, valid police check, NDIS Worker Check and WWCC
- Current driver’s license and own vehicle
