**The role**: You will bring your energy and clinical care experience to establish, develop and evaluate holistic care plans for Residents living in Residential care homes in this part time (0.4FTE) role (4 days per fortnight) based at West Park in Goolwa.

You will promote quality of care through the clinical care assessments, involvement in care planning processes and working in partnership with a multidisciplinary team.

**What you'll do**:

- Develop care plans with a person-centred approach
- Coordinate any specialist assessments and referrals
- Perform ongoing reviews and changing needs of the residents
- Provide oversight of ACFI/AN-ACC process and identify improvements to ensure consistency of practice.
- Accept accountability and responsibility in funding maximisation and proactively engaging in multi-disciplinary team to achieve best care and funding outcomes
- Contribute to ACH Group Quality Management System and commitment to processes of continuous improvement activities, including auditing, and needs analysis

**About You**

The role demands a person with registered nursing qualifications and a current registration certificate, a strong track record in delivering clinical and funding KPIs, and a demonstrated capacity to build effective relationships with key stakeholders.
- Demonstrated significant clinical experience in residential care setting working as an RN and significant experience in a funding role
- Excellent interpersonal and communication
- Excellent documentation skills and attention to detail
- Commitment to customer centric care and quality processes

**You will need**:

- Nursing qualification and current registered nurse registration with AHPRA
- Knowledge and understanding of Aged Care Standards, legislation, and regulations
- To care about supporting older people to live a Good Life.
- Flu & up to date COVID-19 vaccinations (at least 3 doses)
- A** **Police Check for unsupervised contact / vulnerable groups less than 12 months old
