GexaCare is a registered NDIS provider with a focus on quality, compliance and innovation. We are seeking an experienced **Level 3 Support Coordinator** to join our team. This newly created role offers the opportunity to establish and grow Support Coordination services within our organisation.
- **The Role**_
You will hold responsibility for a small caseload of up to 15 participants, providing specialist support coordination for people with complex needs. As the first Support Coordinator at GexaCare, you will be expected to actively build and manage your caseload, develop strong referral pathways, and set high standards of service delivery.
As the service grows, the role will transition into a leadership position, supervising and mentoring a team of Level 2 and Level 3 Support Coordinators.
- **Key Responsibilities**_
- Deliver Level 3 Support Coordination to participants with complex needs.
- Build and manage your own participant caseload.
- Develop and implement service plans that reduce barriers and improve outcomes.
- Establish effective referral pathways with stakeholders, providers and community services.
- Monitor budgets, prepare reports, and ensure compliance with NDIS Practice Standards and the NDIS Code of Conduct.
- Contribute to service development, quality improvement and innovation initiatives.
- Step into a team leader role as demand increases, with responsibility for recruitment and supervision of other Support Coordinators.
- **About You**_
- Tertiary qualification in psychology, occupational therapy, social work, allied health, developmental or health science.
- Registration with a relevant professional body (e.g. AHPRA, AASW).
- Minimum two years’ experience providing Level 3 Support Coordination or similar services for people with complex needs.
- Strong understanding of the NDIS, service systems and compliance requirements.
- Proven ability to establish referral pathways and build caseloads.
- High-level interpersonal, negotiation and advocacy skills.
- Strong organisational skills with the ability to manage competing priorities.
- Current Working With Children Check, National Police Check, NDIS Worker Screening, and valid Australian driver’s licence.
- **Pay & Classification**_
- **SCHADS Level 4.2-4.3 (Full Time)**: $45-$50 per hour (approx. $90,000 p.a. + super).
- **SCHADS Level 5.1+ (Leadership)**: $51-$55 per hour (approx. $98,000-$105,000 p.a. + super), once team leadership responsibilities commence.
