We are currently seeking a dedicated Home Care Package Care Co-ordinator to join our team. Our co-ordinators play a pivotal role in assisting our clients to access services to enable them to remain living safely at home as independently as possible.
The primary purpose of the role is to develop, implement and coordinate services in accordance with the Home Care Package guidelines and ensure the standard of care delivered meets all organization, industry, legislative and professional standards.
**Duties and Tasks-**
- Undertake and understand case management including complex, high and moderate care requirements.
- Conduct initial and ongoing assessments that are person centred and that meet the health, well-being and support needs of individuals and their primary carers.
- Develop and regularly review support plans to ensure clients’ needs and goals are met.
- Complete workplace assessments in accordance with Occupational Safety and Health guidelines
- Monitor home care package costs and ensure services are provided within budget constraints.
- Develop and implement specific strategies to support individuals with complex needs; providing mentoring to Support Workers where appropriate.
- Liaise with the Services Team to ensure optimal service delivery to all clients by supporting them through their journey from admission through to discharge.
- Work collaboratively with Registered Nurses, GP’s and other health/medical officers in relation to clients’ clinical care.
- To ensure day to day delivery of services and programs are consistent with organisational policies and procedures, Government legislation, and quality standards.
- Complete all associated documentation and update all appropriate computer systems in accordance with organisational policies and procedures, the associated Funding Body standards and any relevant accreditation standards.
**Essential-**
- Minimum Certificate IV in Aged Care or Disability, as appropriate.
- Previous experience of compiling individualised Support Plans.
- Possess a thorough knowledge of relevant legislation, standards, government policy direction and modern trends in service provision and the implications for service delivery.
- High level of written, verbal and interpersonal communication skills.
- Effective planning and time management skills.
- Ability to coordinate the planning, implementation and evaluation of services.
- Ability to provide supervision and mentoring to other staff.
- Demonstrated service coordination skills including the ability to work autonomously and as part of a team.
- Decision making and problem-solving skills.
- Completion of NDIS Worker Orientation Module
- Successful Completion of the NDIS Worker Screening Clearance Check
**Salary**: From $41.52 per hour
