**Duties include**:
Provision of community based palliative care services - perform assessments, develop management plans, and review outcomes.

Work closely with GPs and specialists to provide integrated and seamless care.

**Essential criteria**

GP Specialist or CMO

Ideally, you would have experience in a community setting and/or palliative care.

Demonstrate excellent interpersonal, oral and written communication skills.

Ability to work in multi-disciplinary teams.

Ongoing commitment to professional development.

Demonstrate understanding of workplace health and safety and continuous quality improvement
