Credentialed Diabetes Educator

**The Opportunity**

Are you a passionate **Credentialled Diabetes Educator** looking to make a meaningful impact? Purposeful Care is offering an exciting opportunity to join our team and provide high-quality diabetes education services across **Brisbane, Logan, Ipswich, and the Gold Coast**. In this hybrid contracting role, you will deliver diabetes management and education to individuals with diabetes or prediabetes through a mix of community-based, clinic, and Telehealth consultations.

**Key Responsibilities**
- Provide high-quality, person-centred diabetes education and care to adults in community, clinic, and **Telehealth settings**.
- Collaborate with clients, families, and healthcare teams to develop tailored diabetes management plans.
- Deliver education sessions on managing diabetes, healthy lifestyle changes, and effective use of diabetes technology.
- Maintain accurate and timely clinical documentation.
- Stay up-to-date with the latest evidence-based practices in diabetes management.
- Work closely with other healthcare professionals to provide holistic and comprehensive care.

**About You**

To succeed in this role, you’ll need:

- **Qualifications**: Credentialled Diabetes Educator, registered with the Australian Diabetes Educators Association.
- **Licenses**: A current driver’s license is desirable.
- **Skills**: Strong communication skills and the ability to work independently.
- **Availability**: At least 1 day per week, with the potential to increase.
