About the role
The Hospital in the Home (HITH) unit operates a seven day service, inclusive of shift work. The HITH service provides acute, sub-acute and post-acute care to patients in their home delivered through a multidisciplinary team approach. HITH provides admitted hospital care substitution and enables patients greater choice in where their hospital care is received. Through the provision of hospital care at home, patients can receive treatment outside of a tradition hospital bed setting.
Under supervision you will support person and family centred, quality, safe patient care, whilst gaining increased knowledge and understanding the function of the organisation, healthcare teams and the profession of nursing/midwifery.
**Some of the key responsibilities of the role are**:

- Is responsible for own healthcare practice and to the accountable registered nurse/registered midwife for delegated care/intervention.
- Is responsible for accepting delegated tasks/duties, which are commensurate with training and assessed competencies.
- Accepts responsibility to check with a registered nurse/registered midwife when unsure.
- Demonstrates skills appropriate with the level of undergraduate training and competencies.
- Contributes to the care needs of recipients of healthcare services.
- Performs a range of tasks appropriate with the level of training and assessed competencies as delegated by the registered nurse/midwife.
- Works within a healthcare team under the direct supervision of a registered nurse/midwife.

- Ability to demonstrate to follow direction and seek advice from the Registered Nurse, practicing within scope of practice.
- Ability to work as a team member to achieve clinical and organisational goals.
- Ability to resolve issues with a problem solving framework.
- Ability to effectively prioritise and manage own time and exceed consumer expectations, with an enthusiastic, calm and flexible approach.
- Supports and respects the multidisciplinary team, communicating effectively and demonstrating a commitment to achieve common goals.
- A strong understanding of the relevant standards and processes and actively identifies areas for improvement, planning and acting to resolve issues with an awareness of community needs.
- Ability to use clinical information systems relevant to area of practice.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
