**About the Role**:
The Integrated Primary Care (IPC) program is dedicated to improving access to Allied Health and Specialist Nursing services within the primary health care setting across the Yorke Peninsula, Mid and Lower North, and Southern Fleurieu/Kangaroo Island LGAs. The Care Coordinator plays a vital role within a multidisciplinary team, contributing to the effective delivery of the IPC program. This role involves close collaboration with all IPC team members and key stakeholders to support various aspects of the program, including referral processing, data collection, and stakeholder engagement.

**Experience Requirements**:

- High level interpersonal and communication skills (verbal and written) and consultative skills reflected in an ability to engage and collaborate with, a wide range of service providers and community members
- Ability to capture and share information using client record management systems
- Understanding of chronic disease self-management principles and information
- Demonstrated experience in working effectively with people with a chronic disease
- Experience in working with Aboriginal communities and the ability to provide culturally appropriate support.

**Qualification and Other Requirements**:

- Relevant tertiary qualifications (primary health care, nursing, health promotion, public health, community development) or combination of relevant training and experience deemed to be appropriate.
- Drivers Licence
- Current or willing to obtain a Working with Children's Check (WWCC)
