About the Role
We are currently looking for motivated and passionate Assessment Officers to join the supportive and friendly teams at our Brindabilla located in Ashwood on a permanent part-time basis, averaging 48 hours a fortnight.
Key Duties
The assessment officer will be responsible for managing incoming referrals from other agencies, performing telephone and home-based assessments and re-assessments, creating care plans and making outgoing referrals to appropriate agencies for clients and their carers.
The assessment officer also has a role in networking with other service providers in the region and to promote Baptcare’s Social Support Programs within the community.

Key Selection Criteria
Demonstrated ability to complete holistic assessments and reassessments of carer/care recipient needs
Demonstrated ability to develop, implement and evaluate holistic care plans of carer/ care recipient
Highly developed customer service and communication skills.
Demonstrated ability to work both as a member of a team and independently
High level of organizational, planning and reporting skills including negotiation and problem solving skills, decision making, computer skills, literacy and numeracy
Current Australian Drivers Licence
Covid vaccinations plus any booster/s and willingness to have annual flu vaccine
