Support Worker Community Access

The Community Access Support Worker provides essential support to individuals with disabilities, mental health conditions, or other support needs, enabling them to participate in community activities, develop independence, and enhance their quality of life. This role involves assisting clients with daily activities, transportation, social engagement, and skill-building while promoting their overall well-being.
**About You**:
We want someone that is kind, polite, courteous, and eager to learn. Someone who genuinely cares about their clients is eager to listen and assist them in realising their full potential. A genuine desire to offer the highest quality assistance for persons with disabilities, as well as the capacity to create good connections with clients and other stakeholders, is required.
**Key Responsibilities**
- Support clients in accessing and participating in community activities, events, and social outings.
- Provide transport and accompany clients to appointments, recreational activities, and community programs.
- Encourage independence by promoting skill development and engagement in meaningful activities.
- Build positive relationships with clients, families, and support networks.
- Assist clients with personal care, meal preparation, and household tasks if required.
- Implement individual support plans in line with client goals and preferences.
- Maintain accurate records, including progress notes and incident reports.
- Ensure compliance with workplace health and safety policies and safeguarding practices.
- Work collaboratively with colleagues, healthcare professionals, and community organisations to provide holistic support.
**Key Selection Criteria**
**Essential**:
- Previous experience in disability support, aged care, mental health, or community services.
- A genuine passion for supporting individuals to live independently and engage in the community.
- Strong communication and interpersonal skills.
- Ability to work independently and adapt to different client needs.
- Knowledge of person-centred approaches and safeguarding principles.
- Basic computer skills for documentation and reporting.

**Required Certification**:
- First Aid and CPR certification
- Current Working with Children Check
- National Police Check
- Valid driver’s license
- NDIS Worker Screening Check
- NDIS Moduel
