**About The Dementia Centre**

The Dementia Centre, founded by HammondCare nearly 30 years ago, is an impartial provider of research, consultancy and education to the dementia care community. We are dedicated to improving the quality of life for people living with dementia, and also serve families, care teams, clinicians, organisations and the care sector.

**About The Role**:
This role, based in either Hobart TAS or Illawarra Region NSW, is key to providing both pre and post support and counselling to families of loved ones who are moving from an Acute Care setting into a Residential Care Setting. We have two positions available - one in **Hobart, TAS **and one in the **Illawarra Region, NSW**.

This is initially a shorter term contract as it's a pilot scheme, however we expect this role to be extended.

The role aims to reduce emotional distress and promote the wellbeing of the clients and their families.

Key Duties include:

- Provide pre and post support and counselling to families/representatives
- Collection of data to better understand what is required for successful transitions of clients living with dementia from Acute to Residential care
- Facilitate negotiations between families, acute facilities and Aged Care facilities during transition
- Support families and manage expectations of the placement of someone living with dementia.
- Aligning to the Mission of HammondCare - _Improving the quality of life for people in need._

**About you (selection criteria)**

The main requirement for this role is experience working as a Social Worker or Registered Psychologist who has a passion and knowledge of dementia care as well as mental health for older persons.

Other requirements for the role include:

- Strong experience and knowledge of Dementia Care and Mental Health for older persons
- Impeccable communication skills - both verbal and written
- High level of interpersonal skills and empathy.
- Prior experience working in Aged Care would be great, but is not a pre requirement.
