**Classification**: XV16 - ON14
**Salary per hour (exc. super)**: $37.12 - $43.12
**Hours per fortnight**: 64
**Employment Status**: Permanent Part Time

**Role Description**

Bendigo Health Community Care Services has a great opportunity for an experienced Case Manager to join our team and work with Home Care Package consumers and NDIS participants in a community setting.

This challenging but rewarding role will see you supporting people in your local community to remain living at home, providing them with support, flexibility and choice to achieve this.

The Case Manager will be responsible for a caseload of diverse aged care and disability consumers, working holistically and within a goal directed care model to support what care and services are required to promote consumer safety and independence, whilst enabling choice and flexibility in the way their care and support is provided.

**The Team**

The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region. With a range of local and regional programs supporting place-based health promotion, prevention, care and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes. The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.
