**Job Description**:
As a Service Delivery Officer, you will play a pivotal role in ensuring the effective delivery of support services to individuals with disabilities. Your responsibilities will include:
- Conducting comprehensive assessments of clients' needs, abilities, and goals.
- Developing personalized support plans in collaboration with clients, their families, and support networks.
- Coordinating and facilitating access to a range of disability support services, including healthcare, employment assistance, and social activities.
- Providing advocacy and guidance to clients to ensure their rights and choices are respected and upheld.
- Monitoring and evaluating the effectiveness of support programs and interventions, making adjustments as needed to meet evolving needs.
- Maintaining accurate and up-to-date records of client interactions, progress, and outcomes.
- Collaborating with external agencies, service providers, and community organizations to enhance service delivery and access to resources.
- Providing emotional support and crisis intervention as needed to clients and their families.
- Participating in team meetings, case conferences, and professional development activities to enhance service delivery and knowledge sharing.
Skills & Attributes
- Bachelor's degree in social work, disability studies, psychology, or a related field (or equivalent experience).
- Demonstrated experience in disability support services or a related field.
- Strong interpersonal and communication skills, with the ability to build rapport and establish effective working relationships.
- Excellent organizational and problem-solving abilities.
- Commitment to promoting the rights and independence of individuals with disabilities.
Job Type & Salary:
- This is a full-time permanent job.
- The base salary will be AUD 70,000- AUD 75,000 plus superannuation.
- Working Hours: -Availability to work during evenings, weekends, and holidays as required.Why Join Us:
