**The Position**:

- Position Title: Care Coordinator - Transition Care Program
- Classification: Dependant on Qualification
- Salary per hour (exc. super): Dependant on Qualification
- Hours per fortnight: 64
- Employment Status: Part-time Fixed Term
- Other requirements: COVID-19 Vaccination

**Role Description**
- The Care Coordinator will develop collaborative relationships across the health service and community programs to provide high quality coordination, support and timely management of TCP clients. The Care Coordinator acts as the central point of contact for all involved in the care of the client and will utilise strong communication skills to liaise with the multi-disciplinary team and clients/families.

**Selection Criteria**
- Essential : Relevant Allied Health or Nursing degree with associated registration
- Essential : Commitment to ongoing professional development, and ability to meet the requirements of the Bendigo Health Allied Health Credentialing and Scope of Practice document
- Desirable : Demonstrated understanding of aged care and the Transition Care Program, including the care coordinator role in this service
- Desirable : Clinical experience in care coordination/case management/discharge planning with a focus on aged care or rehabilitation
- Desirable : Understanding of Aged Care Assessment Services and Aged Care legislation regarding residential placement
- Desirable : Demonstrated understanding of the impact of hospitalisation on older people
- Desirable : Demonstrated ability to work collaboratively as an individual practitioner within an interdisciplinary team environment including the ability to maintain effective relationships
- Desirable : Sound time management and prioritising skills
- Desirable : Excellent interpersonal, written and verbal communication skills, including excellent computer literacy
